Mary Venus, Mont., had never heard of Billings before being offered a nursing job at a hospital in Billings, Mont., and had never been to the United States. A native of the Philippines, he researched his potential move online, put aside his worries about the Montana winter, and got down to business.

Sugar has been in Billings since mid-November, working in the surgical rehabilitation department at the Billings Clinic, Montana’s largest hospital by population. The Billings hospital is one of several U.S. hospitals looking abroad to address nursing shortages exacerbated by the coronavirus pandemic. The national demand is so strong that there is a steady stream of healthcare professionals waiting for work permits in the United States. More than 5,000 international nurses are awaiting final visa approval, the American Association of International Healthcare Recruiters reported in September.

“We’re seeing a dramatic increase in demand for international nurses,” said Leslie Hamilton-Powers, AAIHR board member and vice president of Avant Healthcare Professionals in Florida.
Before he began recruiting nurses from other countries and then placing them in US hospitals such as the Billings Clinic. Before the pandemic, Avant typically received orders for 800 nurses from hospitals. More than 4,000 such requests have now been received, Hamilton-Powers said.

Billings Clinic will hire 120 more nurses today if available, hospital officials said. Before the pandemic, there was a significant shortage of personnel. The demands and stress of COVID-19 have made this impossible.

Like most hospitals, the Billings Clinic has tried to reduce its staffing shortage by hiring traveling nurses, contract workers who travel to places where the epidemic is needed. The clinic charged up to $200 an hour for its services and at its peak last fall had about 200 traveling nurses as part of its workforce.
The nation’s shortage of nurses has led to such high bills, and members of Congress have asked the Biden administration to investigate unscrupulous agency staff.
Whatever the reason, hospital staffing shortages are unsustainable, said Priscilla Needham, Billings Clinic’s chief financial officer. He pointed out that Medicare doesn’t pay hospitals more if they need to hire more expensive nurses, and doesn’t pay enough when a COVID patient needs to stay in the hospital longer than a typical COVID patient.
From July to October, hospital nursing costs increased by $6 million, Needham said. The Federal Emergency Management Agency and the CARES Act have helped, but he predicted further increases in spending in November and December.

CGFNS International’s Schaffer said foreign-born nurses are interested in the United States for many reasons, including education, career advancement, more money or marriage. For some, the idea of living the “American Dream” prevails, says Avant Hudson.
So far, the rescuers have brought the nurses to the country quickly enough. After being offered and accepted, foreign-born nurses are required to have a final interview with the State Department to obtain a visa, and these interviews are delayed. Powers explained that due to the pandemic, many of the US embassies conducting these interviews are closed or open less than normal hours.
The backlog has been shrinking in recent weeks, but Powers described the delay as a challenge. He emphasized that the nurses-in-waiting in their home countries have passed all the necessary tests to work in the United States.
“It’s very frustrating that nurses are available to come and we can’t get them,” Powers said.

After they arrive, the international nurses in Billings will remain on staff at Avant, but the clinic may offer them permanent positions after three years. Hospital authorities insist that nurses are paid the same as local nurses with similar experience. In addition, the hospital pays Avant.
More than 90 percent of Avant’s international nurses decide to stay with their new colleagues, Hudson said, but the Billings clinic hopes to improve that rate.

Pae Junthanam, a Thai nurse, said she was concerned about coming to Billings after learning that Montana’s population is about 90 percent white and less than 1 percent Asian.
